Registration fees include conference materials, reception, & refreshment breaks.

Payment
We strongly prefer that all payments be made on-line through our secure server.
If you absolutely cannot pay on-line using a credit card, checks should be mailed with the name of the registrant and the conference ("FSM 2012") noted on the check, made out in U.S. dollars and drawn on a U.S. bank, to:

The University of Colorado, Boulder cannot accept purchase orders.
Confirmation
For your convenience, a confirmation screen will appear once you have completed registration. We recommend you print this screen for your records. A confirmation message will also be emailed to you. If you do not see a confirmation screen or receive a confirmation email after you register, your on-line registration has failed and you should try again.
Participant registration will be guaranteed only when all fees have been paid.
Changes
If you need to make a change once you have received your confirmation, please send an email or fax with your name, city/town, and your requested change(s) to confreg@colorado.edu
or fax: +1-303-492-5959. To avoid duplicate registration and additional charges to your account, please do not register a second time.
Cancellations and Refunds
If you must cancel your registration for the workshop, please email or fax your written cancellation notice by April 25, 2012 to confreg@colorado.edu or fax: (303) 492-5959. All cancellations are subject to a $25 processing fee. No refunds will be provided after April 25, 2012.
